Outpatient services are increasingly recognised as an important component of health care provision and may be improved through the application of modern management techniques. We have performed a time and role audit of consultation and waiting times in two medical clinics using different queuing systems: namely, a serial processing clinic where patients wait in a single queue and a quasi-parallel processing clinic where patients are directed to the shortest queue to maintain clinic flow. Data collected were used to construct a (...) computer simulation of patient flows in clinic. Assessment of patient satisfaction in the clinic process was determined using a self-administered questionnaire. Mean waiting time was shorter in the quasi-parallel processing clinic: 26 (SD 17) minutes compared with 36(24) minutes in the serial processing clinic. In the serial processing clinic 61% of patients waited more than 30 minutes compared with 41% in the quasi-parallel processing clinic. In the serial processing clinic 8% of 142 patients surveyed complained of the time spent waiting. The computer simulation we produced was able to determine waiting times with different clinic structures. The simulation showed that reductions in waiting time up to 30% might be achieved by changing our serial processing clinic to a quasi-parallel processing one. Performance of medical outpatient clinics can be improved by examining and changing clinic management. Computer simulation of outpatient clinics offers a means of assessing the impact of such changes on waiting time in clinic and on waiting lists. (shrink)

It is inevitable that some inmates in large state prison systems will suffer from terminal conditions and die while incarcerated. But how those inmates experience that event is primarily controlled by correctional policies and by the prison medical and correctional staff assigned to their care. Compassion for inmates who are dying cannot be legislated or mandated, but humane and compassionate care for the dying can be facilitated or thwarted by legislative and correctional policies, and by the manner in which correctional (...) personnel interpret those policies.Death in New York State prisons is a frequent event, occurring at a rate substantially higher than that in most other states. With a prison population that has risen to 70,000 inmates and with the nation’s highest rate of human immunodeficiency virus infection, more than 2,817 inmates died in New York prisons during the period 1990-1998. In April 1992, in the face of an ever-increasing death rate in its prisons, the New York State legislature passed the Medical Parole Law, a measure designed to permit dying inmates to be released on parole prior to their normal release eligibility date. According to many in the West, the liberalizing effects of North America’s free market ideals will generate equality and justice worldwide. I hold that we should be critical of those who justify imposing liberal democratic ideals on underdeveloped nations by simply suggesting that they promote equality and justice. In the West, entrenched disparities have shaped liberal ideals in ways that make inequality and injustice look natural and normal. Indeed, gender, class, and racial oppression have existed right alongside liberal democratic ideals. (...) If liberal ideals can be molded to make inequality and injustice look natural and normal in the West, then these same ideals can justify economic interactions, technology transfers, military cooperation, and mutual benefit agreements that sustain global inequality and injustice. This dissertation focuses on the contemporary debate over moving from an individualist form of liberalism to one that seeks to accommodate the special claims of various groups in modern society. I deal with authors who examine ways that group dynamics affect the individual. They are worried about whether it is possible or wise to extend individualist liberalism into a group-accommodating liberalism. Presently, it is a matter of deep controversy how liberal democracies ought to interpret and accommodate the social reality and (...) normative claims of racial minorities. ;Race denotes privilege, it precipitates class antagonisms, hostility, and anger between various groups, it motivates misconceptions and misunderstandings. Racism is responsible for both immediate and intergenerational harm. And yet, the members of disadvantaged racial groups see their racial identity as something precious that should be preserved. For many the opportunity to identify with others in terms of a common racial heritage is an important source of pride. This disparity poses special problems for philosophical liberalism. What is the philosophical import of race and racism for various political theories within the contemporary Anglo American tradition of individualist liberalism? How ought the liberal tradition, an individualist thesis, to interpret and accommodate racial identity? ;This dissertation casts the philosophical debate concerning race in a new light. I argue that whether racial identities are compatible with liberal democratic ideals depends on how one defines race and the liberal ideals involved. Hence, I urge a critical stance on the issue of race. I am critical of both classical individualist liberalism, and the stand that groups do not pose any problems in a modern liberal democracy. I argue that we do not get an adequate picture of society from liberal conceptions of race and racism. What this analysis does, then, is call for a synthesis of historical, social, and cultural insights to inform and enrich the philosophical conception of race and racism.

The double membrane of the nuclear envelope is a formidable barrier separating the nucleus and cytoplasm of eukaryotic cells. However, movement of specific macromolecules across the nuclear envelope is critical for embryonic development, cell growth and differentiation. Transfer of molecules between the nucleus and cytoplasm occurs through the aqueous channel formed by the nuclear pore complex (NPC)Abbreviations: NPC, nuclear pore complex; GlcNac, N‐acetylglucosamine; WGA, wheat germ agglutinin. Although small molecules may simply diffuse across the NPC, transport of large proteins and (...) RNA requires specific transport signals and is energy dependent. A family of pore glycoproteins modified by O‐linked N‐acetylglucosamine moieties are essential for transport through the NPC. Recent evidence suggests that the regulation of nuclear transport may also involve the inteaction of RNA and nuclear proteins with specific binding proteins that recognize these transport signals. Are these nuclear pore glycoproteins and signal binding proteins the ‘gatekeepers’ that control access to the genetic material? Logical consistency demands that Mercier and Sperber's (M&S's) argumentative theory of reasoning apply to their own reasoning in the target article. Although they hint that their argument applies to professional reasoners such as scientists and philosophers, they do not develop this idea. In this commentary, I discuss the applicability of argumentative theory to science and philosophy, emphasizing the perils of moral reasoning.

In Marc. gr., classis XI,18 an anonymous florilegium consisting of selected paragraphs of the Second Part of the Fifth Division of the 3rd book of Thomas Aquinas’ Summa contra Gentiles is extant. These paragraphs were excerpted from the Greek translation of the Latin text by Demetrios Cydones and 106 ; ch. 101, § 2 partim; 103; ch. 94, § 3-5; 12-15). The main topic of this text is «fate». An edition of it is offered, and it is argued, on the (...) basis of its similarity with another florilegium Thomisticum of the professed Byzantine Thomist Georgios Scholarios - Gennadios II as well as with some of his writings, that it must be attributed to the same author. It should be probably placed in 1444/53 and regarded as part of Scholarios’ preparation for refuting Georgios Gemistos - Plethon’s Laws II,6, which from 1439 onwards was circulated independently as De fato. In December 2013, the Nonhuman Rights Project (NhRP) filed a petition for a common law writ of habeas corpus in the New York State Supreme Court on behalf of Tommy, a chimpanzee living alone in a cage in a shed in rural New York (Barlow, 2017). Under animal welfare laws, Tommy’s owners, the Laverys, were doing nothing illegal by keeping him in those conditions. Nonetheless, the NhRP argued that given the cognitive, social, and emotional capacities of chimpanzees, Tommy’s confinement (...) constituted a profound wrong that demanded remedy by the courts. Soon thereafter, the NhRP filed habeas corpus petitions on behalf of Kiko, another chimpanzee housed alone in Niagara Falls, and Hercules and Leo, two chimpanzees held in research facilities at Stony Brook University. Thus began the legal struggle to move these chimpanzees from captivity to a sanctuary, an effort that has led the NhRP to argue in multiple courts before multiple judges. The central point of contention has been whether Tommy, Kiko, Hercules, and Leo have legal rights. To date, no judge has been willing to issue a writ of habeas corpus on their behalf. Such a ruling would mean that these chimpanzees have rights that confinement might violate. Instead, the judges have argued that chimpanzees cannot be bearers of legal rights because they are not, and cannot be persons. In this book we argue that chimpanzees are persons because they are autonomous. (shrink)

This paper reexamines the thesis of a split between politics and morality in the political theory of the eighteenth century. At its center is the notion of a “civil society” and the safeguards necessary for its proper functioning. While arguments are drawn from several discourses (economic, juridical, political, ethical), the focus is on the political and ethical reasoning of the age derivative of natural law and dependent upon the free and open exchange of ideas.

This chapter argues that scientific and philosophical progress in our understanding of the living world requires that we abandon a metaphysics of things in favour of one centred on processes. We identify three main empirical motivations for adopting a process ontology in biology: metabolic turnover, life cycles, and ecological interdependence. We show how taking a processual stance in the philosophy of biology enables us to ground existing critiques of essentialism, reductionism, and mechanicism, all of which have traditionally been associated with (...) substance ontology. We illustrate the consequences of embracing an ontology of processes in biology by considering some of its implications for physiology, genetics, evolution, and medicine. And we attempt to locate the subsequent chapters of the book in relation to the position we defend. (shrink)

This paper explores research in educational psychology and learning theory in a search for insights to enhance business ethics training Useful educational principles uncovered are then applied to the development of an ethics training initiative for sales professionals. The paper concludes with suggestions for future research to help enrich business ethics training.

[Edited from Conclusion section:] We have looked at various challenging issues to do with getting connectionism to cope with high-level cognitive activities such a reasoning and natural language understanding. The issues are to do with various facets of generalization that are not commonly noted. We have been concerned in particular with the special forms these issues take in the arena of propositional attitude processing. The main problems we have looked at are: (1) The need to construct explicit representations of generalizations, (...) not just generalize correctly to individual cases; (2) The need to be able to match two or more complex short-term information structures, to enable rapid generalization from recent examples rather than from long-term memories; (3) The need to represent and reason with anomalous combinations of concepts; (4) The need to perform embedded reasoning. This presents special problems for systems using non-concatenative representations (as in mainstream connectionist approaches). We also touched on vague quantification in attitude report complements. Neither this topic nor that of analogies between short-term structures (point 2) has been adequately addressed in the symbolic framework, let alone in connectionism. -/- The opportunities and problems covered are put forward as things worth being optimistic about or pessimistic about, respectively. They are not put forward as decisive arguments for or against connectionism. The hope is that this chapter contributes to a greater understanding of the connectionist/symbolist gap by presenting some unusual issues and by throwing new light on some well known ones. (shrink)

Embodied cognitive science holds that cognitive processes are deeply and inescapably rooted in our bodily interactions with the world. Our finite, contingent, and mortal embodiment may be not only supportive, but in some cases even constitutive of emotions, thoughts, and experiences. My discussion here will work outward from the neuroanatomy and neurophysiology of the brain to a nervous system which extends to the boundaries of the body. It will extend to nonneural aspects of embodiment and even beyond the boundaries of (...) the body to prosthetics of various kinds, including symbioses with a broad array of cultural artifacts, our symbolic niche, and our relationships with other embodied human beings. While cognition may not always be situated, its origins are embedded in temporally and spatially limited activities. Cognitive work also can be off-loaded to the body and to the environment in service of action, tool use, group cognition, and social coordination. This can blur the boundaries between brain areas, brain and body, and body and environment, transforming our understanding of mind and personhood to provide a different grounding for faith traditions in general, and of the historically dualist Christian tradition in particular. A questionnaire on business ethics was administered to business professionals and to upper-class business ethics students. On eight of the seventeen situations involving ethical dilemmas in business, students were significantly more willing to engage in questionable behavior than were their professional counterparts. Apparently, many students were willing to do whatever was necessary to further their own interests, with little or no regard for fundamental moral principles. Many students and professionals functioned within Lawrence Kohlberg's stage four of moral reasoning, the law (...) and order stage. Individualism and egoism remain strong patterns in the moral reasoning of many professionals, but they influence moral reasoning patterns among students to a much greater degree. (shrink)

The common autoimmune disease type 1 diabetes provides a paradigm for the genetic analysis of multifactorial disease. Disease occurrence is attributable to the interaction with the environment of alleles at many loci interspersed throughout the genome. Their mapping and identification is difficult because the disease-associated alleles occur almost as commonly in patients as in healthy individuals; even the highest-risk genotypes bestow only modest risks of disease. The identification of common quantitative trait loci (QTL) in autoimmune disease and in other common (...) disorders, therefore,requires a very close marriage of genetics and biology. Two QTLs have been identified in human type 1 diabetes: the major histocompatibility complex HLA class II loci and a promoter polymorphism of the insulin gene. The evidence for their primary roles in disease aetiology demonstrates the necessity of combined studies of genetics and biology. Their functions and interaction underpin an emerging picture of the basic causes of the disease and direct analyses towards other candidate genes and pathways. The genetic tools used for QTL identification include transgenesis and gene knockouts, whole genome scanning for linkage, mouse congenic strains, linkage disequilibrium mapping, and the establishment of ancestral haplotypes among disease-associated chromosomes.
